CADES
Concord Adult Day Enrichment Services (CADES) began operation in 2005 as a community service to area elderly regardless of religious affiliation. It is a daytime structured program of services and activities for senior adults in need of care, social contact, and supervision because of either physical or mental functioning impairments. CADES provides a safe, nurturing and caring home-like atmosphere for participants, and also provides support, education and respite for caregivers. CADES is licensed by the State of Tennessee Department of Human Services to conduct and maintain an adult day care center.
Monday thru Friday
8:30 am to 5:30 pm
Participants may attend 2, 3,4,or 5 days per week

ADMISSIONS
Admission Criteria
-Ambulatory or semi-ambulatory
-Capable of basic self-care
-Continent or manageable incontinence
-Able to adapt to group setting
Admission Process
Self-referral, physician referral, or community referral. Scheduled assessment interview with participant and caregiver. Medical examination report required upon enrollment.
Services Provided
Staff are trained in dementia specific interventions, including exercise, active and passive games, discussion groups, reminiscent therapy, current events, music therapy, pet visits, arts and crafts, intergenerational contact, gardening, travelogues, guest speakers, educational programs, catered lunch by CUMC, snacks and nutritional support.
Field training site for various area student interns in the fields of nursing, social work and occupational therapy.

$75 Assessment Fee
$71 Daily Fee

DEMENTIA-FRIENDLY WORSHIP
2nd & 4th Sundays
11 AM in Room 207
During this time, Concord United and CADES offers the caregivers the opportunity to attend either the 11 AM Traditional or Contemporary service while their loved ones enjoy a time of devotion and music in a safe and supportive environment. We meet in Room 207 on the second and fourth Sundays of every month.

CAREGIVER SUPPORT GROUP
Caregiver Support Group
Meets the 1st Tuesday of each month
10:00 AM to Noon in Room E 224.
This support group is affiliated with Alzheimer’s TN., Inc., and is dedicated to providing education featuring guest speakers, fellowship, and support for those giving care to an elderly loved one. Refreshments are provided.
